Congenital diaphragmatic hernia, often called CDH, is a serious condition where a hernia (small opening) forms in the diaphragm. This opening can allow organs from the belly to move into the chest, which can limit the space your baby’s lungs need to grow properly.

At the University of Chicago Medicine Fetal and Neonatal Care Center (FNCC), our expert team cares for both you and your baby before and after birth.

How serious is CDH?

When belly organs crowd the lungs, it can cause breathing problems and high blood pressure in the lungs.

The seriousness depends on:

  • Size and location of the hernia
  • Whether the liver moves into the chest
  • How well the lungs develop
  • Any related genetic or heart findings

How do we diagnose CDH?

  • Ultrasound before birth
  • MRI to check your baby’s lung size
  • Heart ultrasound (echocardiogram)
  • Tests for genetic conditions, if needed

How do we treat CDH?

Before Birth

We perform detailed tests, including an ultrasound and MRI.

  • You will meet with a genetic counselor and have an amniocentesis.
  • We will help you plan your delivery.

In some advanced cases, we find that fetoscopic endotracheal balloon occlusion (FETO) is a good option to help the baby's lungs grow before birth. If so, we'll connect your family with a regional center that offers this procedure.

During Delivery

We make sure a NICU team is ready to care for your baby. Vaginal delivery is safe in many cases unless there are other health problems.

After Birth

Your baby's care may include:

Risks

CDH can lead to:

  • Lung and breathing problems
  • High blood pressure in the lungs
  • Feeding and growth challenges, such as failure to thrive
  • Delays in learning or brain development

We review risks with you and offer support every step of the way.
