The University of Chicago Medicine conducts a Community Health Needs Assessment (CHNA) on a regular basis and develops a strategic implementation plan to improve the health of residents in our 12-zip code service area. The Community Health Needs Assessments are a systematic, data-driven approach to determining the health status, behaviors and needs of these residents.

University of Chicago Medical Center Community Health Needs Assessment & Strategic Implementation Plan

Required by the Affordable Care Act, a Community Health Needs Assessment enables communities to identify issues of greatest concern and decide how to allocate resources to address those issues, thereby making the greatest possible impact on community health status.

The 2021-2022 report includes the evaluation of community benefit initiatives since the 2018-2019 UChicago Medical Center Community Health Needs Assessment.

UChicago Medical Center developed the FY 2023-2025 UChicago Medical Center Community Benefit Strategic Implementation Plan to address the health priority areas identified throughout the Community Health Needs Assessment. Our plan provides an overview of efforts by the medical center to assess, prioritize and address the health needs of our community.
