The University of Chicago Medicine conducts a Community Health Needs Assessment (CHNA) on a regular basis and develops a strategic implementation plan to improve the health of residents in our 12-zip code service area. The Community Health Needs Assessments are a systematic, data-driven approach to determining the health status, behaviors and needs of these residents.
UChicago Medicine Community Health Needs Assessment & Strategic Implementation Plan
Required by the Affordable Care Act, a Community Health Needs Assessment enables communities to identify issues of greatest concern and decide how to allocate resources to address those issues, thereby making the greatest possible impact on community health status. UChicago Medicine collaborated with the Alliance for Health Equity to complete a Community Health Needs Assessment of Cook County, which is included as an appendix to the report.

The 2018-2019 report includes the evaluation of initiatives since the 2015 UChicago Medicine Community Health Needs Assessment.
UChicago Medicine developed the 2020-2022 UChicago Medicine Community Benefit Strategic Implementation Plan to address the health priority areas identified through the Community Health Needs Assessment Report. Our plan provides an overview of efforts by the medical center to assess, prioritize and address the health needs of our community.
