The Transplant Institute at UChicago Medicine now offers virtual kidney transplant education with our transplant outreach specialists.
Through virtual outreach sessions patients have access to:
- Obtain kidney transplant information and education tailored to each patient's individual needs
- Initiate the transplant process by completing the transplant intake with the outreach coordinator
- Receive an appointment to start their transplant evaluation
About Virtual Sessions
During a virtual kidney transplant education session, you will meet with a representative from the UChicago Medicine Kidney Transplant Program. You will learn about kidney transplants and have the opportunity to ask questions.
You can also start the kidney transplant process by completing an intake questionnaire and receiving an appointment to begin your transplant evaluation.
You can meet one-on-one or include a spouse, family member(s), or support person(s). The sessions are offered in both video and telephone format and are available in English and Spanish.
Video sessions are conducted using our hospital's secure Zoom account. To participate in a video session with a transplant representative you are required to provide an email address where we can send a link to join the video meeting AND access to either a smartphone or a computer or tablet with a camera. You will receive an email with a secure Zoom video link prior to your session.
If you do not have access to a smartphone or tablet or computer with a camera, you can participate in a phone session. One of our transplant representatives will call patients directly, at the phone number they provide on the date/time of their session.
