Pump Uploads

The diabetes device instruction page outlines different diabetes devices (like glucose monitors and pumps) and the tools you need to upload the data from home and how to share with your doctor. As a reminder, always let your diabetes doctor or diabetes educator know when you have uploaded or if you need help with the upload process. There are also each company's technical support numbers to call for upload help.

For your account data to be “linked” or “shared” with our clinic, you will either need to share your account login information with the clinic, or enter UChicago Medicine's sharing “code” to your account, or accept a sharing invite that the clinic sends to your email.

  • If you need help creating an account or uploading, contact Glooko support



  • Upload device to computer – steps on uploading can be found here and search for your specific blood glucose meter, and by reading the Quick Start Guide

  • Requires a USB cable specific to your blood glucose meter. Please contact the meter company to have the cable sent to you (number located on back of meter) or contact Glooko.


  • Link your account with our clinic by entering the entering the UChicago Medicine ProConnect Code: uofchicago

For the patient if using a smartphone as the receiver (using the DexCom G6 mobile app):



  • Sign into your DexCom Clarity account or sign into the DexCom Clarity mobile app 

  • If you do not know your account information or need helping setting up an account, contact DexCom Technical Support: 


  • Your smartphone should automatically share the data with DexCom Clarity so there is no need to “upload” the device to a computer. 

  • Link your account with our clinic

    • You will need to accept the DexCom email invitation for sharing (Please let the clinic know your correct email address to send you the email invite) 



For the patient using their smartphone as a reader (using the FreeStyle LibreLink mobile app):



  • Log into your account for www.libreview.com 

  • If you do not know your account information or need helping setting up an account, contact Libre customer support


  • Your smartphone should automatically share the data with Librevew.com so there is no need to “upload” the device to a computer. 

  • Link your account with our clinic

    • Enter the UChicago Medicine Practice ID: uofchicago 

    • Or accept the Libre email invitation for sharing (Please let the clinic know your correct email address to send you the email invite) 



For the patient using the Libre reader


Some blood glucose meters are Bluetooth enabled and data can be sent to a mobile device and the logbook can be emailed to your doctor.

If you are unable to download your blood glucose meter, you can use the following links to use a paper logbook and write down numbers before your virtual appointment 



You can also log your blood sugars on an app and send that information to your doctor.

  • Requires a USB cable specific to your OmniPod PDM or DASH. Please contact Insulet (OmniPod company) or Glooko to have the cable sent to you if needed. DASH users should have the USB cable the device is charged with

Another option to upload to t:connect is to download the t:connect app on your cell phone so you do not need to plug your pump into a computer.
