The University of Chicago Medical Center (“UCMC”) covered entities, the University of Chicago Medicine Care Network covered entities, and the University of Chicago Medicine Ingalls covered entities have designated themselves as an affiliated covered entity (“ACE”) for purposes of complying with the Health Insurance Portability and Accountability Act (“HIPAA”).

The University of Chicago Medicine (“UCM”) affiliated covered entity and the UCMC covered entities participate in Organized Health Care Arrangements with the following other covered entities listed below for the purposes of participating in joint activities that include at least one of the following: utilization review, in which health care decisions by participating covered entities are reviewed by other participating covered entities or by a third party on their behalf; quality assessment and improvement activities, in which treatment provided by participating covered entities is assessed by other participating covered entities or by a third party on their behalf; or payment activities, if the financial risk for delivering health care is shared, in part or in whole, by participating covered entities through the joint arrangement and if protected health information created or received by a covered entity is reviewed by other participating covered entities or by a third party on their behalf for the purpose of administering the sharing of financial risk.

The affiliated covered entity and organized health care arrangements allow the UCM organizations to use and disclose your medical information in compliance with HIPAA. The organizations are not agents of each other. They are independent entities responsible for their own activities. We will share your medical information as permitted by HIPAA.
