Annual membership includes all of the components of the comprehensive annual/executive physical and is designed for individuals who want a trusted, high‑touch, relationship‑based primary care home with significantly more access and time than traditional models.

Annual/Executive Physical

We offer comprehensive annual/executive physicals at the Streeterville suite. Opt for a one-time, in-depth health assessment and evaluation built around your personal risk factors, schedule and goals, including:

• Detailed medical history, physical exam and systems review
• Extensive lab panel with advanced cardiac markers and metabolic profiling
• Resting metabolic rate, VO₂ max fitness testing, EKG, body composition via DEXA, plus hearing and skin cancer screening
• A virtual visit with a dietitian and optional genetic and cancer screenings tailored to your risk and interests
• Comprehensive results review with a roadmap for ongoing care and follow-up

The one-day executive physical is designed to compress what typically takes multiple appointments across different sites into a single, coordinated half‑day experience. Longitudinal care and support is included with membership.
