Annual Membership

Ongoing, highly personalized primary care with year-round access and proactive health management.  

Annual membership is ideal if you want a long-term relationship with your concierge medicine doctor and a more personalized, accessible care experience. It includes:  

• Comprehensive annual/executive physical, including routine labs
• Preventive care and wellness planning  
• Ongoing support and follow-up 

Annual/Executive Physical

A one-time, in-depth health assessment tailored to your personal risks, schedule and goals. It includes:  

• Detailed medical history and full physical exam  
• Advanced lab testing (cardiac markers and metabolic profiling)  
• Fitness and diagnostic testing (VO₂ max, EKG, DEXA body composition)  
• Hearing and skin cancer screening  
• Nutritional consultation (with a dietician via a virtual visit)  
• Optional genetic and cancer screenings  
• Personalized results review and care roadmap  

Designed as a streamlined, half-day experience, this evaluation replaces multiple appointments across different locations.  

For a limited time only, if you complete an annual executive physical, the cost can be applied towards the fee for annual membership enrollment.
