Community Health Needs Assessment: Northwest Indiana
UChicago Medicine Northwest Indiana conducts a Community Health Needs Assessment (CHNA) every three years and develops a community health improvement plan to improve the health of residents in our 20-ZIP-code Northwest Indiana (NWI) primary service area. The CHNA offers a systematic, data-driven approach to determining the health status, behaviors and needs of the residents in our NWI community benefit primary service area.
Community Health Needs Assessment & Community Health Improvement Plan
Required by the Affordable Care Act, a Community Health Needs Assessment enables communities to identify issues of greatest concern and decide how to allocate resources to address those issues, thereby making the greatest possible impact on community health status.
