Why Choose Hospital at Home?

Hospital at Home is a new way to receive hospital care. It delivers full hospital-level care in a patient's home instead of having them stay in a hospital.

Research shows that Hospital at Home works well and helps patients feel better and recover faster. Medicare now supports this program. Top medical centers across the country now use this model to deliver hospital care. It's a new approach that brings hospital services into the comfort of a patient's home.

UChicago Medicine's Hospital at Home program is supported by a CMS waiver that lasts through 2030. This allows us to grow this long-running program. It's designed to help patients be more comfortable, move around better, sleep better, and feel better overall. At the same time, we keep hospital-level safety and medical care.

Patients in our program receive these services:

  • In-person nursing visits twice per day at the patient's home
  • Daily telehealth visit with a UChicago Medicine physician
  • Visits with specialists when you need them
  • A daily call from a nurse to make sure your care is going well, plus support for 30 days after you leave the program
  • Medical equipment, including a special tablet with 4G internet so doctors can check on you 24 hours a day
  • Daily meals, if desired by patient
  • Medications, imaging and lab tests at home

Eligibility

Not all patients can participate in UChicago Medicine's Hospital at Home program. If you are eligible, choosing to participate is your choice.

To qualify, you must meet these requirements:

  • You must be receiving treatment for certain conditions. These include heart failure that got worse, COPD that got worse, acute kidney injury, or acute infections.
  • You must live within a 30-minute drive of UChicago Medicine's Hyde Park medical campus.
