The University of Chicago Medicine conducts a Community Health Needs Assessment (CHNA) on a regular basis and develops a strategic implementation plan to improve the health of residents in our 12-zip code service area. The Community Health Needs Assessments are a systematic, data-driven approach to determining the health status, behaviors and needs of these residents.

Required by the Affordable Care Act, a Community Health Needs Assessment enables communities to identify issues of greatest concern and decide how to allocate resources to address those issues, thereby making the greatest possible impact on community health status.

Our community profiles provide overviews of important health factors — including access to care, health behaviors, health outcomes, community resources and more — in several South Side neighborhoods.

Community profiles were compiled using data from the 2018-2019 UChicago Medical Center Community Health Needs Assessment.
