Community Health Needs Assessment & Strategic Implementation Plan - UChicago Medicine Ingalls Memorial
The University of Chicago Medicine Ingalls Memorial conducts a Community Health Needs Assessment (CHNA) on a regular basis and develops a strategic implementation plan to improve the health of residents in our 13-zip code Southland service area. The CHNA is a systematic, data-driven approach to determining the health status, behaviors and needs of these residents.

Required by the Affordable Care Act, a Community Health Needs Assessment enables communities to identify issues of greatest concern and decide how to allocate resources to address those issues, thereby making the greatest possible impact on community health status.
Paper copies of the Community Health Needs Assessments can be found at the hospital’s main entrance and in the lobby of the Professional Office Building. UChicago Medicine Ingalls Memorial developed the FY 2023-2025 UChicago Medicine Ingalls Memorial Community Benefit Strategic Implementation Plan to address the health priority areas identified through the Community Health Needs Assessment Report. Our plan provides an overview of efforts by the medical center to assess, prioritize and address the health needs of our community.
