The University of Chicago Medicine Ingalls Memorial conducts a Community Health Needs Assessment (CHNA) on a regular basis and develops a strategic implementation plan to improve the health of residents in our 13-zip code Southland service area. The CHNA is a systematic, data-driven approach to determining the health status, behaviors and needs of these residents.

UChicago Medicine Ingalls Memorial Community Health Needs Assessment & Strategic Implementation Plan

Required by the Affordable Care Act, a Community Health Needs Assessment enables communities to identify issues of greatest concern and decide how to allocate resources to address those issues, thereby making the greatest possible impact on community health status.

Paper copies of the Community Health Needs Assessments can be found at the hospital’s main entrance and in the lobby of the Professional Office Building. To provide comments and feedback on the Community Health Needs Assessment, Evaluation Report, Strategic Implementation Plan or to receive copies of any of these reports, please email communitybenefit@ingalls.org.
